How It Works

Fill out the digital version of PAGCOR’s official self-exclusion form right here on this page. Provide your personal details, gaming venues you frequent, and choose the exclusion period: 6 months, 1 year, or 5 years.
Upload Required Documents
Attach a scanned copy of your valid government-issued ID and one (1) recent 2×2 colored photo (full face). Family applicants must also upload proof of relationship (e.g., birth certificate, marriage certificate).
